Carry Over requests for staff who have been on Long Term sick but could not apply for the carry over via the e-form process
For staff who have been on Long Term Sickness but were unable to apply for the Carry Over on the trust E-form for any of the following reasons
- 'Criteria not Met' meaning nothing can be added
- more than 5 days balance outstanding (form is capped to 5 days)
please refer this request to your HRBP who will need to submit the request, if approved by DMT, in the format below.
