Staff change forms and processes

A change in staffing is known as an establishment change. This could be an amendment to a role, increased hours or higher salary for example, or a change in a staff member. There are three electronic forms involved in the establishment change process and managers must complete one or more of these forms depending on the nature of the establishment change.

The forms are:

The establishment control form (ECF)
An ECF form must be completed if there is a financial impact to a change in a role or if there is a change in an employee’s terms and conditions. An ECF is always completed when hiring a new or replacement member of staff.

The change of circumstance form (change form)
A change form must be completed when something about a staff member or a role has changed.

The staff leaver form
A leaver form must be completed when a member of staff leaves the Trust.
